Your baby's foreskin is most likely attached to the glans portion of the penis.  This eventually will become more mobile and retractable, but often not before the child is 5 years and sometimes older.  In this region skin cells are shed and tend to accumulate as a white, cheesy or pearly looking discharge.  This is known as smegma and will eventually work its way to the end of the foreskin.  All that is really needed for hygeine purposes is daily gentle washing and rinsing.
